227 UNSCHEDULED ACUTE HOSPITAL ADMISSIONS FROM A COMMUNITY REHABILITATION TEAM – GERIATRIC GIANTS OR “ACOPIA”?

Abstract Background The Integrated Care Team (founded 2018) provides community rehabilitation in the home for frail older adults. Frail adults are at increased risk of hospitalisation, and frailty is associated with length stay mortality. This retrospective cohort study aims to explore acute hospital admissions this from July 2019 December 2021. Methods Patients were identified integrated care team database who had a discharge destination “Acute Hospital”. Hospital IT system data summaries used analyse inpatient Length Stay (LOS), presenting complaints diagnosis. period was Results 67 patients admitted during team, accounting 98 individual admissions. accounted 19.8% discharges period, 77% discharged support. 23.8% (n=16) multiple admissions, majority these(n=14) twice. average age 82.6 years. 58% female. longest LOS 99 days, shortest less than one day. 12.9 days total population. Excluding two outlier stays 82 11.27 days. 32% presented functional decline, 28% falls, 18% shortness breath 19% delirium. Infections diagnosed patients, neurological events 12% bleeding diagnoses 11% patients. 1 patient died admission. Only 12.2% (n=12) 72 hours duration. Conclusion range syndromes complications medical comorbidities. low prevalence short may reflect ongoing role supporting community.
